We are seeking a dedicated and team-oriented Physical Therapist to join our integrated healthcare clinic. The ideal candidate will have a strong background in physical therapy, with experience in an outpatient setting. This role involves assessing patient needs, developing personalized treatment plans, and providing hands-on therapy to help patients regain mobility and improve their quality of life. The Physical Therapist will work closely with patients of all ages, including geriatrics, to facilitate recovery and promote overall wellness.

Responsibilities

• Conduct thorough assessments of patients’ physical abilities and limitations.
• Develop individualized treatment plans based on patient evaluations and goals.
• Implement therapeutic exercises and modalities to enhance recovery.
• Educate patients and their families on treatment plans, exercises, and home care strategies.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Maintain accurate documentation of patient treatments and progress notes.
• Participate in discharge planning to ensure continuity of care post-treatment.

Skills

• Proficient understanding of medical terminology related to physical therapy.
• Knowledge of human physiology and the musculoskeletal system.
• Experience in an outpatient setting is preferred but not mandatory.
• Familiarity with discharge planning processes for effective patient transitions.
• Ability to communicate and effectively lead the PT department.
• Strong interpersonal skills to build rapport with patients and their families.
• Experience working with geriatric populations is a plus.
• Commitment to ongoing professional development in the field of physical therapy.
